ONLINE ADMISSION PROCEDURE

How to apply as a NEW STUDENT in MHCC School Management System (SHOOLISTA)

​

Step 1:

Apply through the SCHOOLISTA Admission Page  (https://premium.schoolista.com/MHCC/Applicant/MHCC.Register.aspx) by filling out the information needed for registration. Then, the One-Time Password and Login credentials will be sent through your email.

For your reference, this is the Department Section Guide.

  1. BED – for Nursery 1 to Grade 10

  2. SHS-BED – for Grade 11 and Grade 12

 

Step 2:

After logging in, fill out all the information in the following categories:

  1. Personal Information;

  2. Family Background;

  3. Siblings/Relatives Information;

  4. Educational Background; and

  5. Reference Information.

 

Then submit your application.

Step 3:

Prepare all the documents needed before going onsite and wait for the email of the Guidance Office for the schedule of your Entrance Examination. Note: Kindly prepare P200.00 (for Preschool to Grade 6 or P300.00 (for Grade 7 to Grade 12) for the Examination Fee.You may opt to pay online through the bank details below and submit the proof of payment in this link (Link: https://shorturl.at/lxV25)

REQUIRED DOCUMENTS:

  1. Original Copy of PSA Birth Certificate

  2. Photocopy of Baptismal Certificate (Roman Catholic)

  3. 2 copies of student pictures (2x2)

  4. Original Copy of Certificate of Good Moral Character

  5. Original Copy of Report Card

  6. Transcript of Records / Form 137 (Grade 2 /High School transferees only)

  7. Certificate of Completion for Kinder (for incoming Grade 1 students)

  8. Photocopy of Certificate of First Communion (Grade 3 /High School)

 

IF APPLICABLE:

  1. Special Power of Attorney or Authorization Letter of Guardianship if the child is NOT in the custody of the legal parents

 

Non-Filipino Citizens:

  1. Alien Certificate of Residence (ACR) issued by the Department of Immigration

  2. Study permits by the DepEd

 

Step 4:

Upon entering the school, submit the Documents in the Registrar’s Office. Then, show the receipt of the entrance examination fee, if you paid through online fund transfers. If not, proceed to the Accounting Office.

 

Step 5:

Proceed to the Guidance Office to take the entrance examination.

​

Step 6:
If you passed the entrance examination, proceed to the Principal’s Office for the interview.

​

Step 7:
If you passed the interview, proceed to the Registrar’s Office for the Enrollment Registration
